About the Role:
The Coordinator - Registered Nurse is primarily responsible for coordinating the day-to-day delivery of person-centred aged care and related community services. The role coordinates participant services, staff and volunteer resources, service documentation, reviews, stakeholder communication and operational requirements to support safe, responsive and sustainable service delivery.
In addition to the coordination function, the position provides registered nursing expertise for Support at Home participants. This includes undertaking clinical assessments and reviews, identifying and escalating clinical risks, contributing to care planning, making referrals, and providing clinical advice to participants, supporters, coordinators and care workers. Direct nursing care may be provided where it is required, authorised and within the Registered Nurse’s scope of practice, although routine direct care is not the primary focus of the role.
Key Duties/Responsibilities
Without limiting the general scope and application of this Position Description, the duties include:
1. Participant Coordination and Service Delivery
Coordinate negotiated, person-centred services in line with participant goals, assessed needs, service agreements, funding arrangements and available resources.
Arrange, implement, monitor and review participant service plans, applying wellness, reablement, independence and supported decision-making principles.
Maintain regular communication with participants and their supporters, ensuring they are involved in decisions and have accessible avenues to provide feedback.
Respond to changes in participant circumstances, service delivery issues and unmet needs, and escalate matters that exceed the role’s authority or available program resources.
Facilitate access and referrals to internal services, GPs, pharmacists, allied health professionals, hospitals, specialist services and other community supports.
Advocate for participants and support coordinated care across providers, including case conferences and transition planning where required.
Receive and respond to complaints and feedback in accordance with policy, implement appropriate corrective action, and keep the Manager informed.
Identify service gaps, changing demand and opportunities for service improvement or growth in the area.
2. Clinical Assessment and Registered Nursing Responsibilities
Undertake comprehensive and targeted nursing assessments for Support at Home participants in their homes or other appropriate settings, using approved tools and clinical judgement.
Assess and monitor relevant areas including general health status and observations, cognition, pain, falls and mobility, skin integrity and pressure injury risk, nutrition and hydration, continence, medication-related risks, chronic conditions, wound needs and other identified clinical concerns.
Identify deterioration, acute risk, safeguarding concerns and unmet clinical needs, and initiate timely escalation or referral in accordance with organisational policy and clinical escalation pathways.
Develop, review and contribute to individualised care and support plans, ensuring clinical risks, participant goals, interventions, monitoring requirements and escalation instructions are clearly documented.
Complete scheduled and event-triggered clinical reviews following incidents, hospital discharge, significant change, new clinical services or concerns raised by participants, supporters or workers.
Provide evidence-based clinical advice and education to participants, supporters, coordinators and care workers, while recognising professional boundaries and the roles of other treating practitioners.

Where required and authorised, provide direct nursing interventions within scope of practice, such as wound care, medication support, continence, catheter or stoma care, health monitoring and other community nursing activities.
Ensure informed consent is obtained and participants’ rights, preferences, dignity, privacy, culture and supported decision-making arrangements are respected.
Maintain current professional knowledge, registration and continuing professional development, and practise in accordance with NMBA professional standards, codes and decision-making frameworks.
3. Workforce Coordination, Supervision and Capability
Coordinate staff and volunteer rosters to match participant needs, worker competencies, continuity preferences, travel requirements, funding and industrial obligations.
Provide day-to-day supervision, guidance and support to care workers and volunteers, addressing operational and interpersonal matters promptly and professionally.
Allocate and delegate care activities only to workers with the appropriate role, competence and support, and provide monitoring or follow-up where clinical delegation is required.
Support recruitment, induction, orientation, probation, supervision, performance feedback and appraisal processes as delegated by the Manager.
Identify workforce competency and training needs, particularly where participants have complex, high-risk or changing clinical needs.
Provide or coordinate practical education and competency support in areas such as clinical observations, skin integrity, falls prevention, medication support, dementia care, infection prevention and recognition of deterioration.
Promote a respectful, accountable and collaborative team culture and facilitate team meetings, individual supervision and effective communication.
4. Clinical Governance, Quality and Safeguarding
Contribute to the implementation of Valmar’s clinical governance framework and ensure clinical care is person-centred, evidence-based, safe, coordinated and appropriately documented.
Operate in accordance with the Aged Care Act, applicable strengthened Aged Care Quality Standards, the Aged Care Code of Conduct, Support at Home program requirements, privacy obligations and organisational policies and procedures.
Identify, report and respond to incidents, near misses, complaints, clinical risks, suspected abuse or neglect, restrictive practices and reportable matters in accordance with legislative and organisational requirements.
Participate in incident reviews, clinical case reviews, audits, file reviews, quality activities and continuous improvement processes, including monitoring whether agreed actions are completed.
Monitor patterns and trends in participant outcomes, incidents, hospital presentations, medication concerns, wounds, falls and other high-prevalence risks, and escalate systemic concerns.
Support safe infection prevention and control practices and promote appropriate antimicrobial stewardship, outbreak and emergency responses where applicable.
Maintain professional accountability for decisions, advice, documentation, delegation and escalation undertaken in the RN capacity.
5. Administration, Systems and Resource Management
Ensure participant records, assessments, care plans, case notes, service agreements, reviews, incidents and other documentation are accurate, current, complete and entered within required timeframes.
Complete required Sandwai processes for service delivery, rostering, auditing, reporting and billing, and support accurate participant accounts and program outputs.
Approve rosters, payroll and other documentation within delegated authority and monitor services against participant budgets and organisational budget parameters.
Contribute to operational, clinical, quality and performance reports as required and provide reliable information to support management decision-making.

Maintain confidentiality, information security and appropriate professional communication across all systems and records.
Monitor the safety and serviceability of equipment used in participant care and report maintenance, replacement or clinical equipment needs.
6. Stakeholder Engagement and Teamwork
Build constructive relationships with participants, supporters, colleagues, health professionals, hospitals, My Aged Care, assessors, service providers and community organisations.
Attend and contribute to team meetings, clinical meetings, case conferences, training and organisational quality activities.
Promote Valmar’s services within the community and support participants to understand available services and how to access them.
Work collaboratively across locations and functions, sharing relevant information to support safe, consistent and coordinated service delivery.
7. Work Health and Safety and Risk Management
Comply with WHS responsibilities and apply risk management principles in office, vehicle, community and participant home environments.
Identify and respond to hazards associated with home visits, lone work, manual handling, infection, aggressive behaviour, environmental conditions and travel.
Complete home and service risk assessments as required and ensure identified controls are communicated, implemented and reviewed.
Participate in emergency, business continuity and incident response arrangements and promptly report hazards, injuries and safety concerns.

Ability to integrate registered nursing accountability with a role whose primary focus is service coordination.
Sound knowledge of person-centred care, supported decision-making, wellness and reablement, safeguarding and contemporary in-home aged care practice.
Demonstrated ability to supervise, support, delegate to and build the capability of a diverse workforce.
Well-developed judgement, problem-solving, conflict resolution and decision-making skills, including the ability to recognise when escalation is required.
Highly developed interpersonal and written communication skills, including clear clinical documentation and professional reports.
Ability to build collaborative relationships with participants, supporters, healthcare professionals and community stakeholders.
Competence with Microsoft business applications and electronic client management, rostering and clinical documentation systems.
Ability to work independently, manage travel and home visits safely, and contribute effectively as part of a geographically dispersed team.
Desirable:
Experience coordinating Support at Home, Home Care Packages, CHSP or comparable community-based programs.
Experience with Sandwai or a comparable client management and rostering system.
Experience in clinical governance, quality auditing, incident review, staff education or competency assessment.
Postgraduate study or formal training in aged care, community nursing, dementia, wound care, continence, palliative care, leadership or care coordination.
Knowledge of the SCHADS Award and/or applicable nursing industrial arrangements, including rostering and entitlements.
Experience working in rural or remote communities and coordinating care across large geographic areas.
Qualifications, Licences and minimum experience requirements:
Current registration with AHPRA as a Registered Nurse
Experience in community, aged care, or primary health nursing
Current Australian driver's license and willingness to travel to clients’ homes
Current First Aid Certificate
Be willing to obtain criminal record checks
